OUR POLICIES

Insurance: We are on the panels of many different insurance companies but please be sure to contact your insurance carrier prior to your child’s visit to be sure you will be covered. Our office will try to make the processing of your claims as streamlined as possible and our billing company works closely with us to do just that.  Please be aware that the ultimate responsibility for your insurance lies with you. It is very important that you know and understand your insurance benefits.  We find that every insurance plan is quite different.  Please contact your insurance carrier if there are any questions regarding coverage.  Also, please check to see if immunizations are covered prior to your visit.  Immunizations will need to be billed differently if they are not covered on your plan.
Be aware that you are ultimately responsible for the payment of your account.  Should you need to make any special arrangements regarding your bill, please contact our office prior to your visit.

Please bring a copy of your insurance card to every visit so we are sure to have the proper billing information.

Co-payments, deductible or co-insurance is due at the time of service.  We accept cash, checks, debit cards and credit cards to facilitate payments.

Treatment Policies: Whenever a child is brought to the clinic treatment is being authorized for that visit regardless of who brings in the child.  If a parent or guardian is not bringing in the child, please bring in written authorization with the child.  IPC may refuse treatment if this authorization is not provided.  We suggest that if you are planning on leaving your child with someone (ie vacation without children) that you fax us an authorization prior to your departure with the name of who will be caring for the child.
An appointment must be scheduled for each child to be seen or who there are questions about.  Every time a child is seen it must be documented in the chart and a billing form must be generated to claim for services.  Even a “peek in the ears” needs to be documented.

Patients arriving later than 10-15 min late may be asked to reschedule their appointment.

Intermountain Pediatric Clinic will not bill a third party, such as an ex-spouse.

Miscellaneous Fees

A $15.00 late fee will be charged if a co-payment is not paid at the time of service.

A $25.00 return check fee will be charged on all returned checks.

A $50.00 service charge may be charged if a confirmed appointment is not kept, including appointments made on the same day.

An after hours fee may be charged to your insurance if you are seen in the clinic after the office is closed.
